Responsibilities:
Provide direct patient care including monitoring vital signs, administering medications, and wound care
Skilled in Trach and vent.
Assist in developing and implementing individualized patient care plans
Collaborate with RNs, physicians, and healthcare team members to ensure optimal patient outcomes
Document patient assessments, treatments, and progress accurately in medical records
Educate patients and families on health management, medications, and discharge instructions
Respond promptly to patient needs and changes in condition, escalating concerns as appropriate
Maintain a safe, clean, and compliant care environment in accordance with healthcare regulations
Requirements:
Active and unrestricted LPN license in the state of Alabama
Graduate of an accredited Licensed Practical Nursing program
Current BLS/CPR certification
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal skills with patients and care teams
Ability to work independently and collaboratively in a fast-paced environment
Compassionate, patient-focused attitude with a commitment to quality care
