Responsibilities:
Administer medications and treatments as prescribed by physicians.
Monitor patient vital signs and report any changes to the healthcare team.
Assist with daily living activities to enhance patient comfort and well-being.
Educate patients and families about health management and wellness.
Maintain accurate patient records and documentation in compliance with regulations.
Collaborate with nurses and physicians to develop and implement patient care plans.
Participate in patient assessments and contribute to care strategies.
Ensure a safe and clean environment for patients and staff.
Requirements:
Valid LPN license in the state of Illinois.
Minimum of 1 year of experience in a healthcare setting preferred.
Strong communication and interpersonal skills for effective patient interaction.
Ability to work collaboratively in a team-oriented environment.
Basic Life Support (BLS) certification required.
Compassionate demeanor with a commitment to patient-centered care.
Proficient in electronic health records (EHR) systems.
Willingness to work flexible hours, including weekends and holidays.
