Job Title: Licensed Practical Nurse (LPN) - Bilingual Spanish
Job Description
This role offers an opportunity for a caring, dedicated, and detail-oriented Licensed Practical Nurse to support a primary care team within a Federally Qualified Health Center. The position focuses on providing high-quality ambulatory care to children and adults, including those living in or near public housing, while promoting continuity of care, patient engagement, and adherence to clinical and regulatory standards in a community-focused, nonprofit environment.
Responsibilities
- Provide compassionate, patient-centered nursing care in an ambulatory primary care setting serving children and adults.
- Support primary care and specialty clinics and programs by addressing barriers to care and promoting continuity of care through proactive patient outreach and engagement.
- Coordinate care management and core coordination activities to help patients navigate healthcare services and follow care plans.
- Handle clinical communications, including answering phone calls and responding to electronic messages, and escalate patient concerns to the registered nurse or provider as appropriate.
- Perform and support clinical testing and procedures within the LPN scope of practice, demonstrating accuracy, safety, and professionalism.
- Ensure the proper functioning of medical equipment and promptly report any issues or malfunctions.
- Manage clinical supplies, including monitoring inventory and restocking exam rooms and clinical areas as needed.
- Maintain exam rooms in a clean, organized, and safe condition, following infection prevention and control protocols at all times.
- Participate in quality assurance and population health initiatives, such as UDS and HEDIS, to monitor and improve service quality and operational efficiency.
- Sustain a neat, orderly workspace and ensure strict adherence to HIPAA, OSHA, CLIA, HRSA, and Joint Commission guidelines.
- Demonstrate respect for patient autonomy and dignity in all interactions, providing culturally sensitive care and clear, professional communication.
- Collaborate effectively with physicians, registered nurses, and other members of the healthcare team to support coordinated, team-based care.
- Fully adhere to organizational policies, procedures, standing orders, and clinical protocols to ensure safe and consistent care delivery.
- Document patient encounters and clinical activities accurately and promptly in the electronic health record system.
- Perform other related duties as assigned to support high-quality primary and specialty care services.
Essential Skills
- Completion of an accredited Licensed Practical Nurse (LPN) program.
- Current, active LPN license in the state of Connecticut.
- Current CPR/BLS certification.
- Minimum of 2-4 years of related clinical experience in an ambulatory healthcare setting.
- Experience in primary care, ambulatory care, or acute care environments.
- Knowledge of infection prevention and control practices.
- Ability to manage care coordination and care management activities in a primary care setting.
- Strong verbal and written communication skills to interact effectively with patients, families, and healthcare team members.
- Strong organizational skills with the ability to prioritize tasks in a fast-paced clinical environment.
- Strong interpersonal and customer service skills to provide a positive patient experience.
- Bilingual fluency in Spanish and English.
- Ability to maintain confidentiality and comply with HIPAA and other regulatory requirements.
- Ability to use or learn electronic health record systems, including Epic.
Additional Skills & Qualifications
- Experience working in a Federally Qualified Health Center (FQHC) or community health setting.
- Familiarity with pediatric patient care in an ambulatory or primary care environment.
- Experience supporting specialty clinics and programs within a primary care setting.
- Understanding of population health metrics and quality programs such as UDS and HEDIS.
- Knowledge of and adherence to OSHA, CLIA, HRSA, and Joint Commission standards and guidelines.
- Demonstrated ability to maintain a neat, organized workspace and exam rooms.
- Strong problem-solving skills and the ability to address barriers to care for diverse patient populations.
- Commitment to serving children, adults, and families in the community, including individuals living in or near public housing.
- Comfort working in a nonprofit organization that offers health, education, and human service programs.

Pay and Benefits
The pay range for this position is $31.25 - $36.00/hr.
