Job Description
As a Licensed Practical Nurse (LPN), you will provide high-quality, patient- and family-centered nursing care while partnering with residents, families, physicians, and interdisciplinary team members to promote optimal health outcomes. You will utilize critical thinking, sound clinical judgment, and excellent communication skills to assess resident needs, coordinate care, and ensure a safe and supportive environment.
Key Responsibilities
- Deliver compassionate, individualized nursing care that respects each resident's culture, values, preferences, and healthcare goals.
- Apply clinical knowledge and critical thinking to assess resident conditions and respond appropriately to routine, complex, and emergent situations.
- Assist physicians and healthcare providers with examinations, treatments, and procedures.
- Conduct resident interviews, screenings, assessments, obtain vital signs, and accurately document findings.
- Perform telephone and in-person screenings while appropriately prioritizing resident concerns.
- Document resident care, assessments, interventions, and communications accurately in accordance with facility policies and regulatory standards.
- Communicate changes in resident condition to providers and collaborate on care plan updates and medication orders.
- Maintain clean, organized, and well-stocked clinical and treatment areas.
- Support a positive, collaborative environment focused on resident safety, dignity, and quality outcomes.
- Maintain compliance with all nursing standards, policies, and infection control procedures.
Additional Job Description
Education
- Graduate of an accredited Licensed Practical Nursing (LPN) program.
Licensure & Certifications
- Current Licensed Practical Nurse (LPN) license in the State of South Carolina or a compact state.
- Current Basic Life Support (BLS) certification through the American Heart Association (AHA).
