Rocky Mountain Adult Day Care is seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide high-quality nursing care and support to our clients. The LPN will work closely with the Registered Nurse (RN), CNAs, caregivers, families, and management to ensure each client receives safe, appropriate, and individualized care.
Responsibilities\n
\n
Provide nursing care within the LPN scope of practice and under RN supervision when required.
\n
Monitor clients’ overall health and identify changes in their conditions.
\n
Check, monitor, and document vital signs, including blood pressure.
\n
Monitor clients with diabetes, high blood pressure, and other chronic health conditions.
\n
Immediately report significant changes in a client’s condition to the RN and management.
\n
Assist with new-client intake assessments and ongoing health reviews.
\n
Assist the RN with developing, reviewing, and updating individualized care plans.
\n
Ensure services are provided according to each client’s care plan.
\n
Maintain accurate, complete, and timely nursing documentation.
\n
Ensure client records contain current assessments, care plans, and other required documentation.
\n
Provide guidance and support to CNAs and caregivers regarding client care.
\n
Observe client care and report staff training or competency concerns to the RN and management.
\n
Complete incident reports accurately and promptly and notify the RN and management of significant incidents.
\n
Communicate appropriately with families, healthcare providers, and other members of the care team.
\n
Follow infection-control, medication-management, safety, and emergency procedures.
\n
Participate in staff meetings, client-care reviews, and quality-improvement activities.
\n
Assist with preparation for state, health department, and other regulatory surveys.
\n
Follow company policies, applicable regulations, HIPAA requirements, and professional nursing standards.
\n
Perform other nursing duties within the LPN scope of practice as assigned.

Qualifications\n
\n
Active and unrestricted Colorado LPN license.
\n
Previous nursing experience required; leadership or team-support experience preferred.
\n
Experience in adult day care, home care, assisted living, or long-term care preferred.
\n
Knowledge of care planning, clinical documentation, incident reporting, and regulatory compliance.
\n
Strong communication, organizational, leadership, and documentation skills.
\n
Ability to maintain confidentiality and comply with HIPAA requirements.
\n
Compensation\n
$28–$35 per hour, depending on qualifications and experience.
