Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and vital signs

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Monitor and report changes in patient status to the registered nurse or physician

• Assist with admissions, transfers, and discharges as needed

• Demonstrate a commitment to delivering exceptional patient care and demonstrating empathy and compassion to our residents

Requirements:

• Currently licensed as an LPN in the state of Michigan

• High school diploma or equivalent required
