We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our home health care team. In this role, you will provide high-quality skilled nursing care to patients in the comfort of their own homes. You will work closely with our RN Case Managers to implement care plans that promote patient independence and recovery.


Responsibilities


  • Clinical Care: Provide bedside nursing care, including wound care, medication administration, and monitoring vital signs.

  • Patient Monitoring: Observe and record patient conditions, reporting any significant changes to the RN or attending physician.

  • Documentation: Maintain accurate, up-to-date electronic health records (EHR) in compliance with state and federal regulations.

  • Education: Teach patients and their families about disease management, medications, and self-care techniques.

  • Collaboration: Participate in multidisciplinary team meetings to coordinate the best possible outcomes for our clients.

Qualifications


  • License: Current, unrestricted LPN license in the state of Indiana.

  • Certification: Valid CPR/BLS certification.

  • Skills: Strong clinical assessment skills and the ability to work independently in a home setting.

  • Requirements: Reliable transportation, a valid driver’s license, and automotive insurance.
