Job Overview
We are seeking a dedicated Permanent Licensed Practical Nurse (LPN) to provide high-quality nursing care in a long-term care setting. The LPN will be responsible for delivering compassionate, patient-centered care while supporting residents' daily clinical and personal needs.
This role requires strong clinical judgment, consistency, and the ability to work effectively within an interdisciplinary care team in a structured care environment.
Key Responsibilities
- Provide direct nursing care in accordance with established care plans and physician orders
- Administer medications and treatments accurately and safely
- Monitor, document, and report changes in resident condition
- Assist with admissions, discharges, and updates to care plans
- Collaborate with RNs, physicians, and interdisciplinary care teams
- Maintain accurate and timely clinical documentation
- Follow infection control and safety procedures
- Support resident comfort, dignity, and quality of life
- Participate in care planning and team communication
Requirements
- Active Licensed Practical Nurse (LPN) license in the state of practice
- Completion of an accredited Practical Nursing program
- Current CPR/BLS certification preferred or required per facility
- Strong communication and teamwork skills
- Ability to work long day shifts (06:45 AM 07:19 PM)
- Prior long-term care experience preferred
