    Summary:

    The Licensed Practical Nurse (LPN) contributes to whole person healthcare by providing evidenced-based nursing skills to care for patients in both the Withdrawal Management and Intensive units.

    Essential Duties & Responsibilities:

    • Timely administer medications to patients and monitor patients for reactions or side effects
    • Maintain accurate, detailed reports and records
    • Record patients' medical information and vital signs.
    • Monitor, record, and report symptoms or changes in patients' conditions.
    • Receive and transcribe written orders from the medical and psychiatric provider
    • Medication Reconciliation process
    • Facilitate effective crisis intervention
    • Assist the RN in consulting and coordinating with healthcare team members to assess, plan, implement, or evaluate patient care plans to include referrals to other levels of care when appropriate
    • Assist the RN in modification of the patient treatment plans as indicated by patients' responses and conditions and collaborate within a multidisciplinary team to achieve best patient outcomes
    • Direct or supervise residential technicians while on shift
    • Provide medical re-education to patients as needed
    • Performs related duties as required.
    • All agency staff are required to attend all mandatory department/agency meetings and trainings*

    Schedule:

    1st Shift: 7:00 AM - 730 PM

    Week 1: Tuesday, Wednesday, Saturday

    Week 2: Sunday, Thursday, Friday

    • Current Connecticut LPN license
