Responsibilities:- Administer medications and treatments as prescribed by healthcare providers.
- Monitor patient vital signs and report any changes to the healthcare team.
- Assist in daily living activities, ensuring patient comfort and safety.
- Document patient care and progress accurately in electronic health records.
- Collaborate with registered nurses and physicians to develop and implement patient care plans.
- Educate patients and families on health management and disease prevention.
- Maintain a clean and organized environment in accordance with healthcare regulations.
- Participate in staff meetings and training sessions to enhance professional development.
Requirements:- Valid LPN license in the state of Maryland.
- Minimum of 1 year of experience in a healthcare setting, preferably in home health or long-term care.
- Strong communication skills and a compassionate demeanor.
- Ability to work independently and as part of a collaborative team.
- Basic Life Support (BLS) certification required.
- Proficient in electronic health record (EHR) systems.
- Commitment to providing high-quality patient care and support.
- Flexibility to work various shifts, including evenings and weekends.
