Job Description

This role offers an opportunity for a caring, dedicated, and detail-oriented Licensed Practical Nurse (LPN) to join a primary care team within a Federally Qualified Health Center (FQHC). The organization provides comprehensive primary healthcare, dental, and mental health services to children and adults in Greenwich and Stamford, with a special focus on individuals and families living in or near public housing. The LPN supports ambulatory primary care and specialty clinics, promotes continuity of care, and contributes to quality and population health initiatives in a nonprofit environment that delivers health, education, and human services across Fairfield County.

Responsibilities

  • Support primary care and ambulatory specialty clinics and programs by providing care management and care coordination that promote continuity of care through proactive patient engagement.
  • Address barriers to care by working closely with patients and the healthcare team to ensure timely access to services and follow-up.
  • Handle clinical communications, including incoming and outgoing phone calls and electronic messages, and escalate patient concerns to the RN or provider as appropriate.
  • Ensure the proper functioning of medical equipment, including routine checks and reporting any issues that may affect patient care.
  • Manage clinical supplies by monitoring inventory levels, restocking as needed, and maintaining readiness in exam rooms and clinical areas.
  • Prepare and maintain exam rooms to meet infection control protocols, ensuring cleanliness, organization, and readiness for patient visits.
  • Participate in quality assurance and population health initiatives, such as UDS and HEDIS, to monitor service quality and operational efficiency.
  • Maintain a neat and organized workspace and ensure strict adherence to HIPAA, OSHA, CLIA, HRSA, and Joint Commission guidelines and standards.
  • Respect patient and client autonomy and dignity at all times, providing compassionate care and maintaining confidentiality.
  • Perform clinical testing and procedures within scope of practice, demonstrating accuracy, professionalism, and attention to detail.
  • Communicate professionally and effectively with the healthcare team, including physicians, advanced practice providers, RNs, and support staff, to coordinate patient care.
  • Fully adhere to organizational policies, procedures, standing orders, and clinical protocols in all aspects of patient care.
  • Utilize the Epic electronic health record system to document patient encounters, clinical communications, and care coordination activities as required.
  • Provide care to diverse patient populations, including pediatric and acute care patients, in alignment with primary care and ambulatory standards.
  • Deliver excellent customer service to patients and families, responding to questions and concerns in a timely and respectful manner.
  • Perform other related duties as assigned to support the smooth operation of the primary care practice and the broader health center.

Essential Skills

  • Completion of an accredited Licensed Practical Nurse (LPN) program.
  • Current LPN license in the state of Connecticut.
  • Current CPR/BLS certification.
  • Minimum of 2-4 years of related clinical experience in an ambulatory healthcare setting.
  • Demonstrated experience in primary care, ambulatory care, or acute care settings.
  • Knowledge of infection prevention and control practices and ability to apply these protocols consistently in exam rooms and clinical workflows.
  • Ability to perform clinical testing and procedures within LPN scope of practice accurately and safely.
  • Strong communication skills, with the ability to interact effectively with patients, families, and members of the healthcare team.
  • Strong organizational skills to manage clinical supplies, exam room readiness, and multiple priorities in a busy ambulatory environment.
  • Strong interpersonal skills that support collaboration, patient engagement, and a positive patient experience.
  • Strong customer service skills, including the ability to respond to patient needs with empathy and professionalism.
  • Ability to respect and support patient autonomy and dignity in all interactions.
  • Ability to adhere strictly to HIPAA, OSHA, CLIA, HRSA, and Joint Commission guidelines, as well as organizational policies and clinical protocols.
  • Ability to manage clinical communications, including phone calls and electronic messages, and escalate issues appropriately.
  • Comfort working with pediatric populations and in acute care situations as part of primary care services.

Additional Skills & Qualifications

  • Experience working in a Federally Qualified Health Center (FQHC) or similar community health setting.
  • Experience supporting care management and care coordination activities to improve continuity of care.
  • Familiarity with Epic or other electronic health record (EHR) systems in an ambulatory care environment.
  • Experience participating in quality assurance and population health initiatives, such as UDS and HEDIS reporting.
  • Bilingual skills in Spanish and English, enabling effective communication with a broader patient population.
  • Experience working with diverse communities, including individuals and families living in or near public housing.
  • Ability to sustain a neat and organized workspace in a fast-paced clinical setting.
  • Commitment to working in a nonprofit organization that provides health, education, and human services to children, adults, and families in Fairfield County.

Work Environment

This position is based onsite at a primary care office within a Federally Qualified Health Center in the Greenwich and Stamford area. The environment is a nonprofit, community-focused setting that offers health, education, and human service programs to children, adults, and families in Fairfield County. The LPN works in an ambulatory primary care practice that may include pediatric and acute care services, using Epic as the electronic health record system and standard medical equipment and clinical supplies. The workplace emphasizes infection prevention and control, adherence to HIPAA, OSHA, CLIA, HRSA, and Joint Commission standards, and maintaining clean, well-prepared exam rooms and a neat workspace. This is a Permanent position based out of Greenwich, CT.

Pay and Benefits

The pay range for this position is $31.25 - $38.50/hr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
