Licensed Practical Nurse

Grand Lake Villa is seeking a dedicated and compassionate Licensed Practical Nurse to join our team. the LPN will provide direct nursing care to residents, ensuring their comfort, safety, and well-being while working under the supervision of a Registered Nurse and Physician.

Responsibilities:

• Provide direct patient care in accordance with individualized care plans.
• Monitor and document residents conditions and report changes to the Physician
• Assist with daily living activities and support resident independence
• Maintain accurate and timely medical records
• Collaborate with healthcare team members to ensure quality care
• Follow all facility policies, procedures, and safety regulations

Qualifications:

• Current and valid LPN license in the state of Oklahoma
• Strong clinical and assessment skills
• Excellent communication and interpersonal skills
• Ability to work effectively in a team environment
• Compassionate, dependable, and patient-focused
