Under the supervision of a RN and in accordance with agency policies and state practice, the Licensed Practical Nurse provides nursing services to individuals and families in their residence under the supervision of a registered nurse.

Responsibilities

Performs prescribed individualized patient care to patients as delegated. Participates in patient case conferences as appropriate; is an active member of the interdisciplinary team. Assists the RN in the preparation, evaluation and modification of the patient''s plan of care as appropriate. Observes, documents and reports to the RN as appropriate, the patient''s physical and emotional needs and reactions to care.

Qualifications

REQUIRED QUALIFICATIONS

  • Must be a Licensed Practical Nurse in Vermont and New Hampshire or hold a compact license.
  • Graduate of an accredited school as a Licensed Practical Nurse

PREFERRED QUALIFICATIONS

  • One year of experience as an LPN in a home health care setting is preferred

Required Licensure/Certifications

  • Unencumbered LPN license in VT and NH or compact.
  • Driver's license, minimum requirements on car insurance, and a clean driving record
