Job Summary

Job type: Full Time

Shift: Days, 8:30 am to 5:00pm

Position Summary:

Under general supervision, this position is assigned to provide skilled nursing care to patients in their homes, following the orders of the attending physician, and to function as a member of the interdisciplinary team.

Responsibilities

Primary Duties

  1. Reassess the nursing needs of the patient regularly. Assist the registered nurse in developing and

implementing changes in the plan of care to meet patient needs.

  1. Provide skilled prevention and rehabilitative nursing procedures as appropriate for the patient's care and safety.

  2. Observe signs and symptoms and report to the Patient Case Manager any changes in the patient's physical or emotional condition.

  3. Teach, supervise, and counsel patients and family members regarding nursing care needs and other related problems of the patient at home.

  4. Utilize community resources and provides for continuity of care through assisting with long range planning and follow-up care.

  5. Maintain clinical records on all patients, including visit notes, progress notes and up to date medication sheets.

  6. Participate in staff meetings, interdisciplinary conferences and inservice programs.

8 Adhere to administrative and clinical policies and procedures.

  1. Demonstrate sensitivity and competence in the care of the ill and/or dying pediatric, adolescent, adult, and geriatric patient.

Qualifications

Minimum Education:

  • Post High School Vocational/Specialized Training

Minimum Experience:

  • 1 year LVN experience

Required Certification/Registration:

  • California LVN License
  • Current CPR Certification
  • Current CA Drivers License
  • Current Automobile Liability Insurance

Preferred Experience:

  • Previous Home Health/Hospice experience.

Preferred Courses/Training

Physical Demands:

  • Moderate physical effort (lift/carry up to 25 lbs.)
  • Occasional prolonged standing/walking
  • Lifting and moving patients
  • Manual dexterity and mobility
  • Occasional reaching, stooping, bending, kneeling and crouching.
