Pay:$48- $50 per hour

Job Description

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team in Santa Maria, CA. This is an excellent opportunity for nurses who value patient-centered care, flexibility, and meaningful work within a supportive healthcare team.

You’ll provide direct patient care in home health and skilled nursing settings, including IV therapy, infusions, central line care, and more.

Key Details

Patient Load: ~5 visits/day (1 admission = 2 visits)
EMR: Netsmart (formerly Allscripts)
Weekend Rotation: Every 3rd weekend
Holiday Rotation: Required
Call: 2–4x/month for weekdays (5 PM–8 AM)
Shift: Flexible, Mon–Fri 8 AM–5 PM (1-hour lunch)
Scrub Color: Any
Orientation: 1.5–2 weeks

Responsibilities

• Provide skilled nursing care in patients’ homes
• Administer IV infusions, antibiotics, and TPN
• Perform central line and PICC line care
• Monitor patient progress and document in EMR
• Educate patients and families on care plans
• Participate in on-call rotation as needed

Qualifications

• Active California LVN License
• BLS Certification
• Minimum 1 year of nursing experience (Home Health preferred)
• Reliable transportation for home visits
• Strong communication & organizational skills
