Job Summary

We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide direct patient care under the supervision of a Registered Nurse. The LPN will support patient care, administer medications and treatments, communicate changes in patient condition, and contribute to the development and implementation of individualized care plans.

Responsibilities

• Provide direct nursing care under the supervision of an RN.

• Assist with personal hygiene and daily patient care needs.

• Participate in the development and updating of patient care plans.

• Administer medications and treatments, including respiratory therapy modalities, and accurately document care provided.

• Follow and enforce infection control policies and procedures.

• Prioritize and organize nursing care effectively.

• Report changes in patients' conditions and incidents to the supervising RN.

• Respond appropriately to stressful, unusual, and emergency situations.

• Maintain positive and professional relationships with patients, families, and staff.

• Provide patient education during hospitalization and upon discharge.

• Assist with orientation and continued learning of new staff members and students.

• Maintain professional and clinical competency through continuing education and training.

• Apply and share new knowledge and clinical skills.

Requirements

• Graduate of an approved Licensed Practical Nurse (LPN/LVN) program.

• Current, active state LPN/LVN license required.

• Minimum 1 year of LPN experience required.

• Current CPR and ACLS certifications required.

• Long-Term Care (LTC) experience preferred.

• Basic computer skills and ability to learn facility-specific software.

• Ability to assess patient conditions and provide effective care using standard nursing practices.

• Strong communication, organization, judgment, and interpersonal skills.
