    Join Summa Home Health as a Licensed Practical Nurse and make a direct impact by helping patients recover safely and comfortably in their own homes.

    As a Home Health LPN, you’ll provide one-on-one skilled nursing care, build meaningful relationships with patients and families, and work alongside a supportive interdisciplinary team focused on positive patient outcomes.

    What You’ll Do

    • Provide skilled nursing care in patients’ homes

    • Follow physician orders and care plans created by the Case Manager

    • Monitor patient conditions and report changes

    • Administer medications and treatments as prescribed

    • Educate patients and families using teach-back methods

    • Help reduce hospital readmissions through patient education and preventative care

    • Complete clinical documentation in accordance with agency and Medicare guidelines

    • Communicate schedule or availability changes with the appropriate team members

    Qualifications

    • Current Licensed Practical Nurse (LPN) license

    • Minimum of 1 year of LPN experience in an acute care setting

    • Home health experience preferred

    • Valid driver’s license and active auto insurance

    • Strong communication, time management, and documentation skills

    • Compassionate, dependable, and patient-focused
