Love & Loyalty Home Care is growing our clinical services program, and we are looking for a dedicated Licensed Practical Nurse to join our team.

This role is ideal for an LPN who is confident in patient care and clinical documentation, organized with details, and comfortable communicating with patients, families, physicians, and healthcare professionals.

You will play an important role in supporting the clinical intake process, coordinating referrals, gathering important patient information, and helping ensure a safe transition into our services.

What Will You Do?

As our Licensed Practical Nurse, you will support the clinical intake process for new patients and contribute to the development of our growing clinical services program.

Your responsibilities may include:

• Managing and processing incoming clinical referrals for skilled and personal care clients.
• Supporting initial clinical intake assessments within the scope of your license and agency protocols.
• Reviewing medical records, hospital discharge documents, and physician orders.
• Gathering and documenting important patient information.
• Communicating with patients and families to understand their needs and concerns.
• Coordinating with physicians, hospital discharge planners, and other healthcare providers.
• Supporting safe and appropriate transitions from hospitals or other healthcare settings into our services.
• Maintaining accurate and timely clinical documentation.
• Following state regulations, agency policies, and clinical standards.
• Communicating important clinical information to leadership and the care team.
• Working closely with our Chief Nursing Officer during onboarding and clinical training.
• Supporting the organization and development of our growing clinical services department.

What Do You Need?

We are looking for an LPN who is:

• A Licensed Practical Nurse with an active, unencumbered LPN license in the applicable state.
• Experienced in clinical nursing and patient care.
• Highly organized and detail-oriented.
• Comfortable reviewing medical records and clinical documentation.
• Able to communicate professionally with patients, families, physicians, and healthcare providers.
• Comfortable using electronic medical records (EMR) and digital communication platforms.
• Able to work independently while collaborating effectively with leadership and the clinical team.
• Reliable, accountable, and able to manage multiple priorities.
• Committed to patient safety, privacy, dignity, and quality care.

Preferred: 1–2 years of clinical nursing experience. Experience in home health, private duty, skilled nursing, or clinical intake is highly preferred.
