Position Overview
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN/LVN) to provide exceptional nursing care in a Long-Term Care (LTC) setting. The ideal candidate will have experience caring for geriatric residents, including individuals with Alzheimer's disease and dementia, while delivering safe, patient-centered care in a collaborative healthcare environment.
Key Responsibilities
Administer medications and treatments as prescribed by healthcare providers.
Perform medication passes accurately while following medication safety protocols.
Monitor residents for changes in condition and promptly communicate findings to the healthcare team.
Provide direct nursing care in accordance with individualized care plans and facility policies.
Complete accurate and timely clinical documentation in the electronic medical record.
Supervise and support Certified Nursing Assistants (CNAs) on assigned units.
Deliver compassionate care to residents with Alzheimer's disease, dementia, and other chronic health conditions.
Maintain infection prevention, patient safety, and quality assurance standards.
Collaborate with physicians, nurses, therapists, and interdisciplinary team members to ensure optimal resident outcomes.
Perform additional duties within the scope of LPN/LVN practice.
Required Qualifications
Education
Graduate of an accredited Licensed Practical Nursing (LPN/LVN) program.
Experience
Minimum 1 year of recent Long-Term Care (LTC) experience required.
Candidates with recent rehabilitation, medical-surgical, or hospital experience will also be considered.
Experience caring for residents with Alzheimer's disease and dementia preferred.
Required Licensure & Certifications
Active New York State LPN/LVN License.
Proof of active New York license required at the time of submission.
Basic Life Support (BLS) certification through the American Heart Association (AHA).

Additional Information
16-week travel contract assignment.
Evening/Night shifts with 5 x 8-hour schedule.
Every other weekend is required.
First-time travelers and local candidates are welcome to apply.
Candidates must successfully complete a medication administration (med pass) test and clinical observation before starting.
Patient ratio: Approximately 1:18–20.
Shift changes are not permitted once an offer has been accepted.
