Job Title: Licensed Practical Nurse (LPN)

Location: Flagstaff, AZ 86004

Pay Rate: $42/hour (Local Candidates Only)


Work Schedule

  • 12-hour night shifts: 6:00 PM – 6:30 AM
  • 36–40 hours per week
  • Full-Time and PRN opportunities available

Position Overview

The Licensed Practical Nurse (LPN) is responsible for administering medications and performing treatments as prescribed by a physician, within the scope of licensure. This role involves providing high-quality patient care, monitoring patient conditions, and collaborating with the interdisciplinary healthcare team to ensure optimal outcomes.


Key Responsibilities

  • Administer medications in various forms (oral, enteral, parenteral, suppository, etc.)
  • Perform treatments including wound care, tracheostomy care, catheter care, and other clinical procedures
  • Observe, monitor, and report the effectiveness of medications and treatments
  • Ensure adequate availability of medical supplies and medications
  • Communicate resident information including medical, social, and functional status
  • Participate in shift reports and ensure continuity of care
  • Collaborate with residents, families, and healthcare team members
  • Complete initial admission nursing assessments
  • Assist in developing Minimum Data Set (MDS) assessments, care plans, and quarterly reviews
  • Maintain accurate clinical and administrative documentation
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
  • Attend and participate in staff meetings

Knowledge Requirements

  • Geriatric and gerontological nursing principles and practices
  • Effects of the aging process
  • Team nursing practices and patient care procedures
  • Pharmacology and medication administration practices
  • Federal, state, and local regulations governing nursing practice and medication control

Skills

  • Ability to observe and report patient behavior and progress
  • Strong understanding of regulatory compliance in nursing practice
  • Effective communication (oral and written)
  • Strong interpersonal and teamwork skills
  • Ability to contribute to care plan development and implementation

  • Comply with all nursing regulations and standards

Requirements

  • Valid Licensed Practical Nurse (LPN) license (AZ)
  • Experience in long-term care or geriatric settings preferred
  • Strong clinical and patient care skills
  • Ability to work night shifts and flexible schedules
