Your Role
- Provide compassionate, high-quality care to residents
- Administer medications and treatments according to plan of care
- Monitor and document patient progress and response
- Communicate effectively with nursing staff and interdisciplinary team members
- Ensure all care is delivered in compliance with facility, state, and federal regulations
Qualifications
- Active New York LPN License
- Strong clinical skills and communication abilities
- Team-oriented with a passion for excellent patient care
- Prior experience in a skilled nursing or rehab setting preferred
