Responsibilities
Provide direct patient care within scope of practice as an LPN under supervision.
Monitor patient status, document vital signs, and report changes promptly.
Assist with medication administration and treatment preparations per protocol.
Uphold safety standards and infection control protocols in all care activities.
Maintain accurate, timely patient records and ensure confidentiality.
Collaborate with multidisciplinary team to coordinate care plans and improve outcomes.
Participate in quality improvement initiatives and track performance metrics.
Assist with scheduling, care coordination, and use of pipelines to project care outcomes.
Ensure compliance with regulatory requirements and facility policies at all times.
Educate patients and families on care plans, self-care, and follow-up steps.
Identify opportunities for professional development and pursue relevant certifications.
Maintain on-site presence and active engagement in a high-visibility, results-driven environment.
Experience/Qualifications
Active LPN license for the respective state; maintain up-to-date credentials.
Adhere to safety, infection control, and HIPAA/privacy regulations.
Provide direct patient care within LPN scope under supervision; document clearly.
Accurate vital signs monitoring and timely reporting of status changes.
Assist with medication administration per facility protocols and policies.
Maintain precise patient records; ensure confidentiality and data integrity.
Collaborate with multidisciplinary team to coordinate care plans and outcomes.

Understand and comply with regulatory requirements and facility policies.
Assist with care coordination and scheduling using available pipelines/tools.
