Position Summary The Licensed Practical Nurse (LPN) plays a critical role in advancing HealthCore Clinic's mission and vision by supporting the delivery of high-quality, accessible healthcare. This position provides direct nursing care, assists healthcare providers with clinical services, and supports care coordination to ensure safe, efficient clinic operations and positive patient outcomes. Working closely with providers, Medical Assistants, and Certified Nurse Aides, the LPN supports patient education, medication management, preventive care initiatives, and continuity of care while maintaining compliance with all applicable federal and Kansas state regulations.
Essential Responsibilities
Respond to patient phone calls and messages, answer questions, process requests, and provide patient education.
Escort patients from waiting areas to exam rooms and prepare them for provider visits.
Obtain and document patient history, vital signs, medications, and allergies in the Electronic Medical Record (EMR).
Prepare and administer treatments as ordered, including immunizations, medications, venipuncture, specimen collection, and hearing and vision screenings.
Supervise and oversee immunizations and medications administered by Medical Assistants and Certified Nurse Aides.
Review and process medication refill requests and prior authorization submissions.
Accurately document patient encounters, phone communications, treatments, and interventions in the EMR.
Notify patients of laboratory and imaging results and respond to follow-up questions.
Support patient self-management and preventive care by providing education and encouraging healthy behavior change.
Assist with oversight and guidance of Medical Assistants and Certified Nurse Aides, including scheduling support and issue resolution.
Maintain clinical equipment, vaccine inventory, and nursing supplies; place orders as needed.
Follow infection control standards, vaccine storage requirements, and universal precautions.
Participate in quality improvement initiatives, health promotion activities, and professional development.
Perform other duties as assigned.
Qualifications & Requirements
Valid and current Licensed Practical Nurse (LPN) license.
One (1) year of previous experience in a medical, hospital, or nursing setting preferred.
Working knowledge of medical terminology and general nursing principles.
Proficiency with Electronic Medical Records and clinical documentation systems.
Ability to communicate effectively and compassionately with vulnerable populations.
Positive, friendly, and professional demeanor.
Strong organizational, time-management, and attention-to-detail skills.
Work Environment & Physical Requirements
Primarily an indoor clinical environment with occasional travel.
Frequent standing, sitting, walking, and use of hands.
Occasional lifting or moving of up to 20 pounds.
Ability to meet the vision, hearing, and communication demands of a clinical setting.
