**About the Role**

We are seeking an experienced and compassionate **LPN (Licensed Practical Nurse)** to join our dedicated care team at Park View. As an LPN, you will play a vital role in providing high-quality patient care, collaborating with our multidisciplinary team of healthcare professionals, and helping our residents achieve their optimum level of health, physical function, and independence.

**Responsibilities:**

* Provide direct patient care, including administering medications, performing skilled nursing interventions, and providing education and support to patients and families
* Collaborate with the care team to develop and implement individualized care plans, monitoring progress and making adjustments as needed
* Communicate effectively with patients, families, and healthcare professionals to ensure continuity of care and optimal outcomes
* Identify and report any changes in patient condition to the care team and physician
* Participate in quality improvement initiatives to reduce the instance of re-hospitalization and improve patient outcomes
