Join Seven Hills Pediatric Center as a Licensed Practical Nurse (LPN) - Children's Congregate Care at our Medically Complex Behavioral Residence in Groton, MA. In this rewarding role, you'll provide hands-on nursing care, health coordination, advocacy, and support in a trauma-informed residential environment where every child deserves to feel safe, respected, and empowered.
You'll work closely with youth, families, medical providers, residential staff, and other professionals to promote optimal health and ensure each child receives the individualized care they need to thrive.

What You'll Do:
Provide individualized nursing and medical support to children and youth with complex medical and behavioral needs.
Assess participants' health needs and coordinate appropriate medical care and follow-up.
Monitor and maintain current medication and treatment orders, including identifying and reporting potential side effects or adverse reactions.
Document nursing interventions, health changes, treatments, appointments, and other required information accurately and timely.
Assist participants with medical appointments and transportation as directed.
Review and maintain plans of care and health records in accordance with regulatory and program requirements.
Communicate medical concerns, recommendations, treatment plans, and upcoming appointments to residential and school-based teams.
Serve as a nursing resource and mentor to Direct Support Professionals, supervisors, and other program staff.
Assist with training staff on medication policies, procedures, and other healthcare-related practices.
Provide direction and supervision to direct care staff to promote safe and appropriate medical care.
Participate in team meetings and collaborate with medical, behavioral health, educational, and residential professionals.
Ensure participants are appropriately observed and supported to maintain their safety and optimal health.
Maintain confidentiality and uphold the dignity, rights, and individuality of every person receiving services.
Complete required training and participate in educational opportunities to maintain and enhance professional knowledge.
Respond appropriately to medical or behavioral emergencies and perform additional duties as assigned.
