Licensed Practical Nurse (LPN) - Community Care

A healthcare organization in New York City is seeking a Licensed Practical Nurse (LPN) to support community-based care across multiple residential sites on Staten Island. This role focuses on providing nursing support, coordinating care, and helping ensure medication and charting tasks are completed accurately for the individuals served.

About the Opportunity:

• Shift: Day shift
• Schedule: Monday through Friday
• Hours: 9am to 5pm
• Setting: In-person, community care

Responsibilities:

• Support RN staff in the delivery of nursing care across assigned community care residences
• Assist with medication management, charting, notes, and follow-up tasks
• Coordinate with the RN buddy system and follow the assigned schedule for each house
• Communicate with staff and care team members to help maintain continuity of care
• Document care activities accurately and help ensure records remain complete and up to date

Qualifications:

• High School Diploma / GED
• Graduate of an accredited Practical Nursing program
• Licensed Practical Nurse credential and active licensure in New York State
• BCLS certification
• Ability to communicate effectively

Desired Skills:

• Associate's and/or Bachelor's Degree in Nursing
• Experience working with individuals with intellectual and developmental disabilities
• Familiarity with electronic medical records or electronic health records
• Experience supporting medication coordination and clinical documentation
