Licensed Practical Nurse (LPN) – Day Shift

Location:Yuma, AZ
Schedule: Monday–Friday | 12-Hour Day Shifts (6:30 AM – 6:00 PM)
Hours: 36 hours one week / 48 hours the following week
Job Type: Contract / Long-Term Care Assignment

Position Overview

"Navitas Healthcare, LLC" is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join a respected long-term care and geriatric healthcare facility in Yuma, Arizona. This role is ideal for an experienced nursing professional who is passionate about delivering high-quality patient care and supporting elderly residents in a collaborative healthcare environment.

The LPN will work under the supervision of a Registered Nurse (RN) or physician and may function as a charge nurse when assigned.

Key Responsibilities

  • Administer medications and treatments in accordance with physician orders and LPN scope of practice
  • Monitor, assess, and document residents’ conditions and responses to treatment
  • Provide direct patient care and assist residents with Activities of Daily Living (ADLs)
  • Perform clinical procedures including wound care, catheter care, tracheostomy care, and enteral feeding support
  • Assist with resident admissions, assessments, care planning, and clinical documentation
  • Communicate effectively with residents, families, physicians, and interdisciplinary healthcare teams
  • Supervise and support CNAs and other support staff as needed
  • Maintain accurate and timely nursing documentation
  • Assist with infection control initiatives and quality improvement programs
  • Participate in shift reports, staff meetings, and ongoing training sessions
  • Support orientation and training of newly hired staff members

Qualifications

  • Active Licensed Practical Nurse (LPN) license in the State of Arizona
  • Strong knowledge of geriatric and long-term care nursing practices
  • Experience with medication administration and patient treatment procedures
  • Excellent communication, organizational, and documentation skills
  • Ability to work effectively in a team-oriented healthcare environment
  • Previous long-term care or skilled nursing facility experience preferred

Requirements

  • Current and active Arizona LPN license required
  • Ability to work 12-hour day shifts with alternating weekly schedules
  • Strong commitment to patient care, safety, and professionalism
