Schedule: Monday–Friday | 12-Hour Day Shifts (6:30 AM – 6:00 PM)
Hours: 36 hours one week / 48 hours the following week

Position Overview
"Navitas Healthcare, LLC" is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join a respected long-term care and geriatric healthcare facility in Yuma, Arizona. This role is ideal for an experienced nursing professional who is passionate about delivering high-quality patient care and supporting elderly residents in a collaborative healthcare environment.
The LPN will work under the supervision of a Registered Nurse (RN) or physician and may function as a charge nurse when assigned.
Key Responsibilities
- Administer medications and treatments in accordance with physician orders and LPN scope of practice
- Monitor, assess, and document residents’ conditions and responses to treatment
- Provide direct patient care and assist residents with Activities of Daily Living (ADLs)
- Perform clinical procedures including wound care, catheter care, tracheostomy care, and enteral feeding support
- Assist with resident admissions, assessments, care planning, and clinical documentation
- Communicate effectively with residents, families, physicians, and interdisciplinary healthcare teams
- Supervise and support CNAs and other support staff as needed
- Maintain accurate and timely nursing documentation
- Assist with infection control initiatives and quality improvement programs
- Participate in shift reports, staff meetings, and ongoing training sessions
- Support orientation and training of newly hired staff members
Qualifications
- Active Licensed Practical Nurse (LPN) license in the State of Arizona
- Strong knowledge of geriatric and long-term care nursing practices
- Experience with medication administration and patient treatment procedures
- Excellent communication, organizational, and documentation skills
- Ability to work effectively in a team-oriented healthcare environment
- Previous long-term care or skilled nursing facility experience preferred
Requirements
- Current and active Arizona LPN license required
- Ability to work 12-hour day shifts with alternating weekly schedules
- Strong commitment to patient care, safety, and professionalism
