The Licensed Practical Nurse (LPN) plays a critical role in supporting the medical and behavioral health needs of individuals enrolled in the CORE program. As part of a multidisciplinary team, the LPN provides medication support, health monitoring, and care coordination to ensure safe, effective, and integrated treatment. This role focuses on improving client stability, promoting medication adherence, and supporting overall wellness.
Primary Job Responsibilities:
Clinical & Nursing Services
Administer and monitor medications as prescribed by the Psychiatrist or Medical Provider
Conduct basic health assessments, including vital signs and symptom monitoring
Observe and document client responses to medications and report concerns to the clinical team
Assist with medication education, adherence support, and side effect management
Support psychiatric evaluations and follow-up appointments
Care Coordination & Support
Collaborate with therapists, case managers, and psychiatric providers to support treatment plans
Coordinate with pharmacies, primary care providers, and other external providers as needed
Assist in bridging physical and behavioral health needs for clients
Support transitions of care and follow-up after hospitalizations or higher levels of care
Documentation & Compliance
Maintain timely and accurate documentation in the EHR (e.g., Credible EHR)
Complete nursing notes, medication logs, and required assessments in accordance with Medicaid and CORE guidelines
Ensure compliance with Georgia DBHDD and organizational standards
Adhere to all HIPAA and confidentiality requirements
Client Engagement & Education
Provide education to clients and families on medication, wellness, and disease management
Support clients in understanding their treatment plans and health needs
Encourage engagement in services and adherence to care recommendations
Service Delivery
Provide services in clinic or via telehealth
Team Communication & Supervision
Participate in team meetings, case reviews, and care coordination discussions
Communicate effectively with supervisors and interdisciplinary team members regarding client status and needs
Follow direction from supervising RN, Medical Provider, or Clinical Leadership
Qualifications:
Active Licensed Practical Nurse (LPN) license in the state of Georgia
Experience working with behavioral health populations preferred
Knowledge of medication administration and monitoring
Strong documentation and organizational skills
Ability to work independently and as part of a multidisciplinary team
Preferred:
Experience in community-based behavioral health or CORE services
Familiarity with Medicaid documentation and compliance requirements
Experience with EHR systems (e.g., Credible EHR)
Bilingual or experience working with diverse populations
