Unit Highlights
The Discharge Hospitality Center (DHC) Licensed Practical Nurse (LPN) facilitates a safe, efficient, and patient-centered discharge process for patients awaiting departure from the hospital. The LPN provides individualized discharge education and reinforces instructions to ensure patients and caregivers understand medications, follow-up appointments, and post-discharge care. Responsibilities include completing accurate documentation, assessing patients for any additional care needs while in the lounge, coordinating safe and appropriate transportation home, and maintaining a safe, comfortable environment until discharge. The LPN collaborates closely with the interdisciplinary team to promote a seamless transition from hospital to home while supporting patient satisfaction and safe post discharge outcomes.
Full-time Monday-Friday
Key Responsibilities
As a Clinical Nurse, you will provide skilled, compassionate care to patients and families across a spectrum of needs-from routine to highly complex. You will assess evolving patient conditions, identify actual and potential health concerns, and develop appropriate care plans. Interventions may be delivered directly or coordinated through collaboration with other members of the nursing team
