Qualifications

Required- Evanston, IL

• Associate degree or higher

• 1+ years of experience in ANY of the following:
  • Home Health Care
  • Home Health
• 1+ years of experience in ANY of the following:
  • Electronic Medical Record (EMR)
  • Other EMR
  • Practice Partner EMR

• Valid Licensed Practical Nurse (LPN) in the state of Illinois (IL)
• Valid Certified Wound Ostomy and Continence Nurse (CWOCN)
• Valid Regular Driver License (DL) in the state of Illinois (IL)

Key Responsibilities Manage and deliver patient care plans collaboratively within a team Provide wound care services to patients in the coverage area Maintain accurate and timely charting and documentation Use EMR software and mobile devices for patient care management Drive to patient locations within the assigned coverage area 

Schedule and Shift Details Monday – Friday, 8:30 AM – 5:00 PM; no weekends, holidays, or on-call; typically sees 6-7 patients per day Travel Must live within 30 minutes of coverage area; approximately 20% driving to patient locations
