Join our team as a Licensed Practical Nurse

As a Licensed Practical Nurse (LPN), you will play a vital role in providing quality healthcare services to patients in their homes. Your responsibilities will include conducting assessments, administering medications, providing wound care, and assisting with various treatments under the supervision of a Registered Nurse (RN). Your compassionate nature and clinical skills will contribute to the overall well-being of patients, promoting their recovery and ensuring their comfort. Duties include but are not limited to:

  • Provide excellent, patient-centered, and quality care
  • Ensure compliance with all agency policies, procedures, and protocols
  • Maintain accurate patient records and report changes in status or condition
  • Support patient recovery and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy
  • Participate in patient admissions, discharges, and transfers
  • Conduct charting as required in accordance with the agency policy and procedure
  • Participate in other duties as assigned by the Nurse Manager

Qualifications:

  • Current dual state license as a Licensed Practical Nurse in Illinois & Wisconsin
  • Current valid driver's license
  • Experience working in a home health setting a plus
  • Current CPR certification
  • Experience using an Electronic Medical Records system preferred
