Licensed Practical Nurse (LPN)

Location: New York 14456
Pay: $1,136 – $1,285/week

• Full-Time (5x8-hour evening shifts)

• Every other weekend (EOW) required

• Holidays required

• Provide direct and indirect nursing care to adult and geriatric residents under RN supervision

• Assess and monitor resident physical and psychosocial conditions; report changes to RN/team

• Assist in developing and updating individualized care plans

• Administer treatments and support daily care in accordance with policies and procedures

• Collaborate with interdisciplinary team to ensure quality resident outcomes

• Maintain accurate documentation and ensure a safe, supportive care environment

• Active NY LPN License (Required)

• BLS Certification (AHA or RQI only)

• Minimum 1 year of LPN experience in an LTC setting

• High School Diploma or equivalent required

• Strong clinical, communication, and teamwork skills
