Position Overview
Bond Health is currently hiring a Licensed Practical Nurse (LPN) for a part-time opportunity in Queens, NY. The LPN will support individuals living in an Intermediate Care Facility (ICF) Group Home by promoting health, safety, and wellness.
Key Responsibilities
Supervise and assist direct support staff with personal care and daily living activities.
Administer medications according to established procedures.
Monitor infection control practices and maintain first aid supplies.
Document medical findings and report significant changes to the RN Supervisor.
Communicate with healthcare providers and family members regarding medical concerns.
Provide treatments including wound care, G-tube feedings, blood glucose monitoring, urinary drainage maintenance, and range-of-motion therapy as ordered.
Monitor vital signs, weights, and other health indicators.
Ensure compliance with federal and state regulations.
Attend medical appointments and required training.
Perform other duties as assigned.
Qualifications
Current unrestricted New York State Licensed Practical Nurse (LPN) license.
AHA or equivalent CPR certification.
Experience with individuals with intellectual and developmental disabilities preferred but not required.
Excellent written and verbal communication skills.
Ability to work independently and as part of a team.
Valid New York State driver's license highly preferred.
