Shift: 7:00 PM – 7:00 AM | Tuesday, Friday, Saturday & Sunday
Position Overview
Bond Health is currently hiring a Licensed Practical Nurse (LPN) for a part-time opportunity in Queens, NY. The LPN will support individuals living in an Intermediate Care Facility (ICF) Group Home by promoting health, safety, and wellness.
Key Responsibilities
- Supervise and assist direct support staff with personal care and daily living activities.
- Administer medications according to established procedures.
- Monitor infection control practices and maintain first aid supplies.
- Document medical findings and report significant changes to the RN Supervisor.
- Communicate with healthcare providers and family members regarding medical concerns.
- Provide treatments including wound care, G-tube feedings, blood glucose monitoring, urinary drainage maintenance, and range-of-motion therapy as ordered.
- Monitor vital signs, weights, and other health indicators.
- Ensure compliance with federal and state regulations.
- Attend medical appointments and required training.
- Perform other duties as assigned.
Qualifications
- Current unrestricted New York State Licensed Practical Nurse (LPN) license.
- AHA or equivalent CPR certification.
- Experience with individuals with intellectual and developmental disabilities preferred but not required.
- Excellent written and verbal communication skills.
- Ability to work independently and as part of a team.
- Valid New York State driver's license highly preferred.
