    Description

    The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a Skilled Nursing Facility under the supervision of a Registered Nurse (RN) or Director of Nursing (DON). The LPN is responsible for administering medications, monitoring resident conditions, coordinating care, and ensuring compliance with facility policies and regulatory standards while promoting the highest quality of resident care.

    Essential Duties & Responsibilities

    • Administer prescribed medications and treatments in accordance with physician orders.
    • Monitor residents' health status and report changes in condition to the RN, physician, or nursing leadership.
    • Perform resident assessments and contribute to the development and implementation of care plans.
    • Document nursing interventions, resident responses, and changes in condition accurately and timely.
    • Provide wound care, catheter care, tube feeding management, and other nursing procedures as permitted by state regulations.
    • Supervise and delegate tasks to Certified Nursing Assistants (CNAs) as appropriate.
    • Respond to resident emergencies and provide appropriate nursing interventions.
    • Ensure compliance with infection prevention and control policies.
    • Educate residents and families regarding treatment plans, medications, and health maintenance.
    • Participate in interdisciplinary care plan meetings and resident reviews.
    • Maintain compliance with federal, state, and facility regulations, including CMS guidelines.

    Qualifications

    • Active LPN license in good standing.
    • Graduate of an accredited Practical Nursing program.
    • Current CPR/BLS certification preferred.
    • Previous experience in Skilled Nursing, Long-Term Care, Rehabilitation, or Post-Acute Care preferred.
    • Knowledge of nursing practices, medication administration, and resident care standards.
    • Strong clinical assessment and critical-thinking skills.
