  Responsibilities:<\/span><\/span><\/b>
  <\/p>

  • Provide patient care according to client’s care plan and needs<\/span><\/span>
    <\/li>
  • Perform wound care treatments and assist clients with medication management<\/span><\/span>
    <\/li>
  • Teach the client and/or family members about care and support skills<\/span><\/span>
    <\/li>
  • Communicate regularly with RN Case Managers, Supervisors, and the Care Team<\/span><\/span>
    <\/li><\/ul>

    Qualifications:<\/span><\/span><\/b>
    <\/p>

    • Licensed as a Practical Nurse / LPN in the state of Minnesota<\/span><\/span>
      <\/li>
    • Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License<\/span><\/span>
      <\/li>
    • Demonstrated ability to work autonomously<\/span><\/span>
      <\/li>
    • Prior Nursing experience in a TCU, Hospice, or Home Health setting<\/span><\/span>
      <\/li>
    • Experience with Electronic Medical Records (EMR) software is preferred<\/span><\/span>
      <\/li><\/ul>
