Position OverviewAs a Home Health LPN, you will provide skilled nursing care to patients in their homes while collaborating with the interdisciplinary care team to ensure high-quality, individualized care.You’ll work independently in the field - but never alone. Our Clinical Supervisors and operational staff are readily available to support you.
Responsibilities- Deliver skilled nursing care in accordance with the established plan of care
- Monitor and assess patient conditions during home visits or shifts
- Communicate changes in patient status to the Clinical Supervisor
- Provide patient and caregiver education regarding medications, treatments, and disease management
- Accurately document patient care and clinical observations
- Coordinate care with other healthcare team members as appropriate
- Maintain compliance with agency and state guidelines
Schedule Options- Day, evening, overnight shifts available
- Weekday and weekend opportunities
- Full-Time, Part-Time, PRN, Per Diem
- Choose your own hours based on availability

Qualifications- Active LPN license (Pennsylvania and/or Compact License)
- Ability to work independently in a home setting

- Reliable transportation
