Licensed Practical Nurse (LPN) – Home Health – Per Visit / PRN

Penn Home Health Care is seeking compassionate Licensed Practical Nurses (LPNs) to provide skilled nursing services to patients in their homes under the supervision of a Registered Nurse and in accordance with physician orders.

Service Areas:
Philadelphia, Delaware, Chester, Montgomery, Bucks, and Berks Counties, Pennsylvania

Position Type:
Per Visit / PRN

Responsibilities:
• Provide skilled nursing visits in accordance with the patient's plan of care.
• Monitor patient status and report significant changes to the supervising RN and physician.
• Provide medication education and reinforcement of nursing teaching.
• Assist with disease management and chronic condition monitoring.
• Perform treatments and interventions as permitted under Pennsylvania nursing regulations and agency policy.
• Document patient care accurately and timely.
• Participate in interdisciplinary care coordination.

Qualifications:
• Current Pennsylvania Licensed Practical Nurse license in good standing.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• One year of clinical nursing experience preferred.
• Home health experience preferred but not required.
