An organization serving individuals experiencing homelessness is seeking a Licensed Practical Nurse (LPN) in Brooklyn, NY. This role provides on-site nursing support in a shelter setting, with a focus on medication management, clinical screening, and care coordination for residents.
About the Opportunity:
- Shift: Day shift
- Schedule: Monday - Friday
- Hours: 8:00am - 4:00pm
- Setting: In-person, homeless shelter
Responsibilities:
- Provide on-site nursing services and support residents under the direction of nursing leadership.
- Manage medication systems, including medication administration records, safe storage, refills, and medication education.
- Administer medications, injections, treatments, and other provider-ordered procedures.
- Obtain vital signs, complete nursing screenings, and document care in the electronic medical record.
- Coordinate with medical, case management, and administrative teams to support resident care and follow-up.
Qualifications:
- Active New York State Licensed Practical Nurse (LPN) license.
- American Heart Association (AHA) BLS certification.
- 2-3 years of clinical experience in a psychiatric, mental health, behavioral health, substance use, or primary care setting.
- Excellent oral and listening skills with the ability to communicate effectively with residents and staff at all levels.
- Knowledge of Microsoft Office, especially Word and Excel.
- Familiarity with electronic medical records documentation.
Desired Skills:
- Experience working with homeless individuals.
- Knowledge of eClinicalWorks EMR.
- Phlebotomy skills.
- Background as a medical assistant or CNA.
- Bilingual communication skills.
