An organization serving individuals experiencing homelessness is seeking a Licensed Practical Nurse (LPN) in Brooklyn, NY. This role provides on-site nursing support in a shelter setting, with a focus on medication management, clinical screening, and care coordination for residents.

About the Opportunity:

  • Shift: Day shift
  • Schedule: Monday - Friday
  • Hours: 8:00am - 4:00pm
  • Setting: In-person, homeless shelter

Responsibilities:

  • Provide on-site nursing services and support residents under the direction of nursing leadership.
  • Manage medication systems, including medication administration records, safe storage, refills, and medication education.
  • Administer medications, injections, treatments, and other provider-ordered procedures.
  • Obtain vital signs, complete nursing screenings, and document care in the electronic medical record.
  • Coordinate with medical, case management, and administrative teams to support resident care and follow-up.

Qualifications:

  • Active New York State Licensed Practical Nurse (LPN) license.
  • American Heart Association (AHA) BLS certification.
  • 2-3 years of clinical experience in a psychiatric, mental health, behavioral health, substance use, or primary care setting.
  • Excellent oral and listening skills with the ability to communicate effectively with residents and staff at all levels.
  • Knowledge of Microsoft Office, especially Word and Excel.
  • Familiarity with electronic medical records documentation.

Desired Skills:

  • Experience working with homeless individuals.
  • Knowledge of eClinicalWorks EMR.
  • Phlebotomy skills.
  • Background as a medical assistant or CNA.
  • Bilingual communication skills.
