    Schedule: Monday - Friday, 8:15AM - 5:00PM with on-call requirements

    Agency: Mercy Hospice

    SUMMARY 

    The Licensed Practical Nurse I (LPN), Hospice provides comprehensive skilled care as ordered by the attending physician/medical director by following the order sets created by the RNCM. The LPN I educates patients and their family members and ensures the safety of the patient. The LPN I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations, as outlined by the state. 

    KEY RESPONSIBILITIES 

    • Provides skilled nursing care in a patient’s home as ordered by the attending physician/medical director 

    • Performs care by following order sets and goals made by RN 

    • Observes and monitors patient conditions 

    • Notifies RNCM of any changes in patient condition 

    • Reconciles medications prescribed by the physician 

    • Counsels patient and family on the disease/progression and how to manage 

    • Incorporates patient and family in development of plan of care 

    • Listens to patient and family members 

    • Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines 

    • Documents all aspects of treatment, assessments, and patient education 

    • Communicates with scheduler any changes outside the normal 

    • Participates in all on-call requirements and case conferences 

    • Attends in-service trainings and mandatory agency meetings 

    • Maintains a professional appearance as a representative of the company 

    • Understands and practices agency policies and procedures 

    • Other duties as assigned 

    QUALIFICATIONS 

    • One year of experience as an LPN in an acute care setting is strongly recommended. Hospice care experience is desired. 

    • Ability to create positive impressions and communicate with a variety of people. Maintain effective communication with patients, families, physicians and co-workers. Ability to remain calm, have patienceand be accommodating. Compassionate and caring while working with patients and families. 

    • Knowledgeable on nursing best practices. Ability to make appropriate nursing judgments. Ability to identify a situation and handle it with the best possible solution. Detail-oriented and observant. 

    • Disciplined style of work ethic. Ability to prioritize and be timely. Works efficiently. 

    • Ability to follow directions and work as a team member. 

    • Valid driver’s license and auto insurance with your name as a listed driver. 

    EDUCATION AND CREDENTIALS 

    • Licensed Practical Nurse with current license in the state of employment. 

    • Maintains active LPN license in state of employment
