Job Description
A healthcare organization in New York City is seeking a Licensed Practical Nurse (LPN) to join their team in a residential group home setting in Manhattan.
About the Opportunity:
- Shift: Day shift
- Schedule: Monday through Friday
- Hours: 10:00 AM to 6:00 PM
- Setting: OPWDD residential / group home
Responsibilities:
- Provide medical oversight and support to residential program participants
- Assist with assessment, planning, and implementation of medical care
- Administer and monitor medications in accordance with physician orders and agency policies
- Train and support direct care staff on medical protocols and procedures
- Coordinate medical appointments, follow-up care, and ongoing healthcare needs
- Maintain accurate clinical documentation and communicate with interdisciplinary teams
- Participate in team meetings and provide medically related in-services to staff
- Ensure compliance with OPWDD, agency, and regulatory requirements
Qualifications:
- Active NYS Licensed Practical Nurse (LPN)
- Strong communication, organization, and documentation skills
Desired Skills:
- Experience working with individuals with Intellectual and Developmental Disabilities
- Medical-Surgical experience
- Experience in Residential, Group Home, OPWDD, Behavioral Health, or Long-Term Care settings
- OPWDD experience
- Medication Administration and Staff Training experience
- Care Coordination and Case Management experience
