Position Summary The Licensed Practical Nurse (LPN) plays a key role in delivering high-quality, in-scope nursing care as part of an interdisciplinary medical team. This position supports patient assessments, clinical procedures, education, and care coordination to ensure a positive and safe patient experience.

Key Responsibilities

  • Assist physicians and RNs with patient examinations and treatments.
  • Administer medications and injections as prescribed.
  • Obtain and monitor vital signs, complete medication reconciliation, and document clinical updates.
  • Perform routine procedures such as lab specimen collection and EKGs.
  • Conduct nurse visits, including wound care, IV therapy, vaccinations, BP checks, patient education, oxygen walk-tests, EKGs, and catheter care.
  • Provide patient and family education on conditions, treatments, and preventive care.
  • Conduct health screenings, immunizations, and focused assessments.
  • Support scheduling of appointments, procedures, and follow-up care.
  • Coordinate referrals to outside providers and facilities.
  • Maintain accurate and timely medical documentation.
  • Collaborate with the interdisciplinary care team to support comprehensive patient care.
  • Participate in training, quality improvement, and professional development activities.
  • Foster a safe, welcoming, and patient-centered clinic environment.
  • Uphold confidentiality, professionalism, and ethical standards.

Required Qualifications

  • Current Oregon LPN licensure.
  • Completion of an accredited LPN program.
  • High School Diploma or equivalent.
  • Eligible to provide services under federal healthcare programs, not excluded by OIG/GSA.
  • Proficiency with electronic health records and basic computer applications.
  • Strong organizational, multitasking, and communication skills.
  • Ability to work independently and collaboratively.
  • Proficiency in written and spoken English.
  • Ability to meet KHP immunization requirements.
  • Ability to pass a background check and drug screen.
  • BLS certification or ability to obtain within 180 days of hire.

Preferred Qualifications

  • Bilingual in English and Spanish.
