Licensed Practical Nurse (LPN) – Long-Term Care (LTC)

Location - Vestal, NY

Duration - 13 weeks

Position Overview:
Seeking a dependable and compassionate Licensed Practical Nurse (LPN) with long-term care experience to provide high-quality nursing care in a skilled nursing facility. The ideal candidate will be experienced in medication administration, resident assessment, and working collaboratively with interdisciplinary care teams.

Required Qualifications

  • Active LPN License
  • Minimum 1 Year of Long-Term Care (LTC) Experience
  • Ability to Successfully Pass a Medication Administration (Med Pass) Test
  • Ability to Successfully Complete Clinical Observation Requirements

Key Responsibilities

  • Administer medications and treatments safely and accurately
  • Perform medication passes in accordance with physician orders and facility policies
  • Monitor residents and report changes in condition to the appropriate healthcare providers
  • Complete timely, accurate, and compliant clinical documentation
  • Deliver direct nursing care to residents in a long-term care setting
  • Assist with resident assessments and care planning
  • Supervise and support Certified Nursing Assistants (CNAs) on assigned units
  • Ensure compliance with all regulatory and facility standards
