Shift Length: 12 Hours
Guaranteed Hours: 36 Hours/Week
Compensation
- Local Rate: Up to $40/hr (based on experience)
- Travel Rate: Up to $49/hr (based on experience)
Position Overview
"Navitas Healthcare, LLC" is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join a dedicated long-term care and skilled nursing team in Grand Junction, Colorado. The ideal candidate will possess strong clinical and patient care skills, with experience in long-term care, rehabilitation, or post-acute settings preferred.
This position requires flexibility for night shifts and the ability to thrive in a fast-paced healthcare environment while delivering exceptional resident-centered care.
Key Responsibilities
- Provide direct nursing care in accordance with individualized care plans and physician orders
- Administer medications and treatments safely and accurately
- Monitor, assess, and document resident conditions and report any significant changes
- Perform routine nursing procedures including wound care, vital signs, and basic assessments
- Assist residents with activities of daily living (ADLs) while ensuring dignity, comfort, and safety
- Collaborate with nurses, providers, caregivers, and interdisciplinary teams to support quality patient outcomes
- Maintain accurate and timely clinical documentation
- Adhere to infection prevention, safety, and regulatory guidelines
- Support care planning initiatives and resident-focused treatment goals
- Respond promptly and professionally to resident needs during assigned shifts
Qualifications
- Active Licensed Practical Nurse (LPN) license in the state of Colorado required
- Graduate of an accredited Practical Nursing program
- Previous experience in long-term care, skilled nursing, rehabilitation, or post-acute care preferred
- Current BLS/CPR certification preferred
- Strong clinical, assessment, and documentation skills
- Ability to work independently and collaboratively within a healthcare team
- Excellent communication and interpersonal skills
- Compassionate, dependable, and patient-focused approach to care
- Ability to manage multiple patient assignments efficiently in a fast-paced environment
