Licensed Practical Nurse (LPN) – Long-Term Care (LTC)
Location: Platte, South Dakota
Assignment Length: 13 Weeks
Shift: Nights (5:30 PM – 6:00 AM)
Schedule: 12-hour shifts | Every Other Weekend (EOW)
Position Overview
"Navitas Healthcare, LLC" is seeking a compassionate and dependable Licensed Practical Nurse (LPN) for a Long-Term Care facility in Platte, SD. The ideal candidate will have strong geriatric nursing experience and be comfortable working night shifts in a skilled nursing environment, delivering high-quality, patient-centered care.
Key Responsibilities
- Provide direct nursing care to long-term care residents in accordance with individualized care plans
- Safely administer medications and treatments as prescribed
- Monitor, assess, and document patient conditions and changes
- Perform wound care, vital signs, and routine nursing procedures
- Assist residents with activities of daily living (ADLs) including hygiene, mobility, and feeding
- Report significant changes in condition to the RN and care team
- Maintain accurate and timely documentation in the electronic medical record (EMR) system
- Follow all infection control and facility safety protocols
- Support resident comfort, safety, and quality of life during overnight care
- Collaborate effectively with CNAs and interdisciplinary team members
Qualifications
- Active LPN license (South Dakota or Compact license required/preferred)
- Graduate of an accredited Practical Nursing program
- Prior experience in long-term care or skilled nursing preferred
- Current BLS certification preferred
- Strong clinical assessment and documentation skills
- Ability to work night shifts and every-other-weekend rotation
- Reliable, compassionate, and patient-focused approach to care
- Strong communication and teamwork abilities
