    Licensed Practical Nurse

    Treatment Coordinator

    Nassau County, NY

    ATC Healthcare Services, LLC of Nassau County has an IMMEDIATE NEED for an experienced Licensed Practical Nurse (LPN) to serve as a Treatment Coordinator in a Skilled Nursing and Rehabilitation Facility located in Freeport, NY.

    This is an excellent opportunity for a dedicated LPN with strong clinical, organizational, and communication skills to play a key role in coordinating patient treatments, ensuring quality care, and supporting positive patient outcomes in a fast-paced rehabilitation environment.

    Schedule: Saturdays & Sundays, 7:00 AM – 3:00 PM

    Essential Duties & Responsibilities

    • Coordinate and implement patient treatment plans as directed by physicians and Registered Nurses.

    • Administer medications, treatments, and nursing interventions in accordance with physician orders and facility policies.

    • Monitor patient conditions and promptly report changes to physicians, Registered Nurses, and other healthcare professionals.

    • Perform routine assessments and document patient responses to treatment.

    • Review and update treatment records, care plans, and medical documentation.

    • Schedule and coordinate appointments, consultations, diagnostic testing, and follow-up care.

    • Communicate treatment goals and patient progress with residents, families, and healthcare team members.

    • Assist with wound care, dressing changes, catheter care, and other clinical procedures as assigned.

    • Ensure compliance with infection control standards, patient safety protocols, and regulatory requirements.

    • Participate in interdisciplinary care conferences and contribute to care planning discussions.

    • Educate patients and families regarding treatment plans, medications, and health management strategies.

    • Maintain confidentiality of patient information in accordance with HIPAA regulations.

    • Assist with quality assurance and performance improvement initiatives.

    Qualifications

    • Active New York State Licensed Practical Nurse (LPN) License required.

    • Current CPR/BLS Certification required.

    • Minimum one (1) year of nursing experience preferred.

    • Experience in long-term care, rehabilitation, home care, outpatient, or clinical settings preferred.

    • Knowledge of medical terminology, treatment protocols, and nursing practices.

    • Proficiency with Electronic Medical Records (EMR/EHR) systems.

    • Strong organizational, communication, and interpersonal skills.

    • Ability to prioritize tasks and manage multiple responsibilities in a fast-paced environment.

    • Excellent critical thinking and problem-solving skills.
