Licensed Practical Nurse (LPN) MDS Coordinator

Camellia Health and Rehabilitation is seeking an experienced and detail-oriented Licensed Practical Nurse (LPN) MDS Coordinator to join our clinical team in Mobile, Alabama. This position plays a vital role in ensuring accurate resident assessments, timely MDS completion, regulatory compliance, and quality reimbursement while working collaboratively with our interdisciplinary team.

Responsibilities

• Complete and submit MDS assessments accurately and within required timeframes

• Coordinate the MDS process with the interdisciplinary team

• Develop and update resident care plans based on assessment findings

• Ensure compliance with CMS, Medicare, Medicaid, and state regulations

• Review clinical documentation to support accurate reimbursement

• Monitor assessment schedules and submission deadlines

• Participate in quality improvement initiatives and survey readiness

• Communicate effectively with nursing staff, therapy, physicians, and families as needed

Qualifications

• Current Alabama Licensed Practical Nurse (LPN) license in good standing

• MDS experience in a skilled nursing or long term care setting preferred

• Knowledge of PDPM, Medicare, Medicaid, and the RAI process

• Strong assessment, documentation, and organizational skills

• Excellent communication and computer skills

• Ability to work collaboratively with an interdisciplinary team
