Job Description:
 
We are seeking a highly skilled and empathetic Hospice Care Specialist to join our team. The ideal candidate will possess extensive experience in providing end-of-life care, demonstrating both clinical expertise and a deep understanding of the emotional and psychological needs of patients and their families.
 
Key Responsibilities:
 
·        Deliver comprehensive hospice care to patients/clients in their homes.
·        Provide emotional support to patients/client and their families.
·        Administer medications and treatments as prescribed, ensuring patient comfort and symptom management.
·        Conduct regular assessments and report to DON for adjustments to the care plans as needed to meet changing patient/client needs.
·        Educate patients/clients and families about the hospice care process, addressing any questions or concerns.
·        Maintain accurate and detailed patient records in compliance with regulatory standards.
 
 
Qualifications:
 
- Licensed Practical Nurse (LPN) license.
- Minimum of 3 years of experience in hospice or palliative care.
- Strong clinical skills and the ability to manage complex medical conditions.
- Excellent communication and interpersonal skills.
- Empathetic and compassionate nature with a strong commitment to patient-centered care.
- Ability to work independently and as part of a collaborative team.
- Proficiency in electronic medical records (EMR) systems.
