Licensed Practical Nurse/Licensed Vocational Nurse, Long-Term Care, Nursing, Patient Care
Responsibilities
- Provide direct nursing care to residents in accordance with physician orders and care plans.
- Administer medications and treatments safely and accurately.
- Monitor and document resident conditions and report changes promptly.
- Assist with admissions, discharges, and ongoing resident assessments.
- Collaborate with RNs, physicians, and interdisciplinary team members.
- Maintain accurate medical records and documentation.
- Follow infection control, safety, and facility policies.
- Promote a safe, respectful, and resident-centered environment.
Requirements
- Active New York LPN License.
- Minimum 2 years of recent Long-Term Care (LTC) experience.
- Current BCLS Certification.
- NY DHHS Background Check required.
- National, State, and County background checks.
- 10-Panel Drug Screen.
- Current health requirements:
- 2-Step TB Test
- Hepatitis B
- MMR Immunization
- Tdap
- Varicella Immunization
