We are seeking a Licensed Practical Nurse (LPN) to join our supportive and mission-driven team. The LPN will play a vital role in providing high-quality clinical care, supporting daily health needs, and fostering a safe and positive environment for the individuals we serve.
Provide skilled nursing care in accordance with state regulations and company policies.
Administer medications and treatments as prescribed.
Monitor and record vital signs, symptoms, and patient progress.
Collaborate with physicians, RNs, and caregivers to ensure continuity of care.
Educate patients and families on health management and wellness.
Maintain accurate, timely, and confidential medical records.
Respond to medical emergencies with professionalism and urgency.
Active Licensed Practical Nurse (LPN) license in the state of [Insert State].
CPR and First Aid certification (or willingness to obtain).
Strong clinical and critical thinking skills.
Compassionate, reliable, and dedicated to patient care.
Excellent communication and teamwork abilities.
Experience in home health, community health, or long-term care is preferred but not required.
